Traveling the world over, when I was young.....one had to not just have a PassPort......but an immunization "passport" also.

The immunization passport was yellow.....and listed all the shots one had.

Let me tell you.....to go from the US to Pakistan in 1965.....one had to get a "butt load" of shots....

There were Immunization Check Points at points on egress and ingress to the USA....along with PassPort Customs check points....

I don't think anything like this is done anymore????

It's not the poor who are refusing or missing vaccinations, it is primarily the wealthy areas. (see link above) Some vaccinations cannot be given until a child is old enough, though, so they are vulnerable to catch it from adult carriers. Elderly can sometimes lapse in getting booster shots simply because they're less mobile and it doesn't occur to them.

California is the first to provide a map breakdown by area, so we have to use Calif as an example. Calif's vaccination rate is terrible, but not the worst.

If you are an affluent Scientologist who can afford to put your kids in private schools then maybe you can get away with exercising your "Parental Rights" to not vaccinate but what if you aren't so fortunate and your kid is in a public school with 40-45 children in a classroom and a high percentage of non vaccinated children from 3rd world countries? If there is going to be a provision to allow people to not vaccinate then there should be an equal provision for parents to place their children in other schools with less risk.

That is. indeed ironic. I read once - I think it was an article in The Atlantic which was about exactly the point you are making above - that the highest levels of the "new unvaccinated" were among parents who had money, good jobs and education. One of the areas (again going from memory here) that had the highest rates of unvaxed was marin county.
